How to Register:

1.    TEAM REGISTRATION FORMAT! Team Registration Deadline: Wednesday, April 30, 2008 for all interested teams. If entering more than one team in a level, indicate which team should be priority #1, #2, #3 (per level) of getting into the league. Multiple representatives from the same school must communicate with each other prior registering teams.

2.    League openings will be filled using the following format:

a.    Initially each school will be allowed up to one team in each level (1-Gold, 1-Silver, 1-Bronze), indicate this as priority team #1.

b.    Then #2 priority teams from schools with multiple teams per level will be added.

                                        i.      Teams with complete rosters, signed waivers, and all fees paid will be added first.

                                        ii.      Followed by a lottery selection for the remaining openings (if needed).

c.     Then #3 priority teams from schools with multiple teams per level will be added.

d.    Hopefully we will be able to accommodate all interested teams; the 2007 league was full so Register Early!     

3.     All teams must have an adult (18 yrs. or older) to serve as the coach/manager.

4.    Players must be listed on the roster form provided and fees turned in by Wed., April 30, 2008. All participants must have a parent/guardian’s signature on the roster or a parent/guardian signed individual registration form attach to the team’s roster in order to participate.

5.    Each non-resident participant must pay a $20 fee.  If you live in the Village of Holmen, it only costs $10 for residents. All fees must be paid or the registration will not be accepted (roster/registration form with the money).

6.     Divisions: There will be three divisions available, GOLD, SILVER, and BRONZE.  Please use a different form for each division and team.  You may photocopy this form. 

There will be a MANDATORY RULES MEETING for the Manager’s/Coaches on Monday, May 19, 2008 at 6 p.m. at Holmen Village Hall, 421 Main St., Holmen. League schedules will be distributed at the meeting. If you cannot attend, please contact Mike Brogan at 526-2152 or Terry Dahmen 534-7768.
